Moving?

If you are moving within Acton or Boxborough...

1. A parent/guardian needs to complete a Proof of Residency Affidavit form.  If needed, a hard copy can be obtained in the Counseling Dept.

2.  Make a copy of your new lease or Purchase & Sale Agreement, which shows parent/guardian name and address.

3. Give both documents to Kerry Lewis (klewis@abschools.org) in the Counseling Department. 

If you are moving out of town...

1. A parent/guardian should notify your counselor of your planned move. Please indicate the student's last day of school.

2. A parent/guardian should complete Section 1 of the Withdrawal Form. (If needed, a hard copy can be obtained in the Counseling Dept.)This makes your move official and also gives the school permission to forward your school records to the new school.

3. Before the student's last day, the student should notify each of their teachers of their move and ask them to complete Section II of the Withdrawal form.

4. Section III of the form needs to be completed before the student leaves school. Make sure all library books are returned. Bring your Chromebook and charger to the main office.
