This page will give you some tips for starting your own Google Form.
Things to remember:
You will probably want a "What is your name?" question. (I always forget this one!)
Have someone test your form before you make it public to see if the answers you get are the answers you expect and want!
Don't be afraid to change questions half way through to make sure they are giving you the answers you want. But note that this changes the validity of your data.
Don't mess with the page in the Responses sheet where your data comes in!
