Sports physicals

Physicals must occur on or after April 15th, 2022 to cover the 2022-2023 school year. Families must submit a physical form signed by a medical physician prior to participation by a student-athlete. 

Families should submit their student's physical via email to jasterk@a2schools.org

Families may also submit paper copies to the main office on the first floor at Slauson once school begins on August 29th.
