2026-2027 Sports Physical Forms
Physicals must occur on or after April 15th, 2026 to cover the 2025-2026 school year. Families must submit a physical form signed by a medical physician prior to participation by a student-athlete.
Families should submit their student's physical via email to Co-curricular Director at Forsythe@aaps.k12.mi.us
Families may also submit paper copies to the main office at Forsythe.
