Sports physicals

Physicals must occur on or after April 15th, 2024 to cover the 2023-2024 school year. Families must submit a physical form signed by a medical physician prior to participation by a student-athlete. 

Families should submit their student's physical via email to Athletic Director, powelld@aaps.k12.mi.us. 

Families may also submit paper copies to the main office at Forsythe once school begins.
