Registration Instructions

 Step 1 for Registration: (A parent/guardian must complete registration)

1. 1.  Go to FamilyID.com

   2.  Click Login, click for families & students.

   3.  If you are new to FamilyID, click "Sign Up" as a family or individual and enter the requested information. If you already have a FamilyID account, "Log In" by entering your email address and password.

   4. Type your Organization's name in the search box, Walpole High School Athletics. Click search.

   5. Click on the Organization name in Search Results.

   6. Click on the name of the Program for which you want to register.

   7. Complete the registration form and hit "Save and Continue".

   8. Click submit to complete the registration.

   9. Once your registration is complete, you will receive an email confirmation from registrations@familyid.com. You can view and update your registrations at any time by going to your Dashboard.

Step 2

Bring to the Athletic Office a copy of your current physical (must be within 13 months), please include the sport your student plans to participate in & the year of graduation. Along with the physical, please drop off the completed and signed Concussion Protocol, which can be found under the Athletic Trainer tab on the WHS Athletic page.

Step 3

Online payment or a check payable to the Town of Walpole.  

Step 4 (Once per school year):

Fill out the Concussion Training Notification Form to verify completion of both Concussion Training for Students and Concussion Training for Coaches and Parents

Impact Testing with the Athletic Trainer, Ally Cromwell
