MN License Application Submission Instructions for Parent Education Candidates
After your last grade is posted, and you completed all of your licensure requirements (coursework, student teaching), complete the PELSB application. Then follow the submission instructions below.
There are two steps in the MN tier 3 license application process:
Step A) Internal Process within U of M and
Step B) Mailing your complete application packet to PELSB via US mail.
Step A: Internally within U of M (Do this first)
Once you complete all of your licensure coursework and your MA degree is posted, email following attachments to famed@umn.edu.
1) In your email to famed@umn.edu
✔ In the subject line of your email, enter
“First Name, Last Name, Parent Education, Minnesota Teacher License Application”
ex: John/Jane Doe, Parent Education, Minnesota Teacher License Application
✔ Your email text should read:
o First Name Last Name
o University of Minnesota ID number
o Phone Number
o Your U of M Program Name: Parent Education
o MN License(s) you are applying for: Parent and Family Education
o My MN License status: choose one and type
“I am a 1st time applicant applying PELSB online” or
“I am a 1st time applicant applying using a PELSB paper application” or
“I hold an MN teaching license (ex, substitute, tier 2, tier 3) applying using a PELSB paper application”
2) Attach the following items to your email to famed@umn.edu.
✔ Parent Education License Application Cover Memo to OTE
✔ Signed Transcript Release Form
✔ Section 6: Verification of Completion of a State-Approved Licensure Program
o Type your name on the top of the page
o Enter the Student Teaching/Practicum in Section 6
o Leave the rest of Section 6 blank
o Save this document as a fillable pdf “First Last Name Section 6” (do not lock this pdf form)
Parent & Family Education program will forward your email along with the attachments to the U of M Office of Teacher Education (OTE).
Step B: Mail your complete application packet to PELSB. (Do this once you received the items highlighted in red from Office of Teacher Education: OTE).
Your application packet should include the following items:
Option 1: If using a PELSB paper application
✔ Sections 1-5 of Application for a Tier 3 Minnesota Educator License Form
o Visit the Minnesota Professional Educator Licensing and Standards Board (PELSB) web site at https://mn.gov/pelsb. Tier 3 Application
✔ Signed Conduct Review Statement (included in the license application section 5)
o Your signature on the conduct review statement must be signed in black or blue ink (original signature)
✔ PELSB Fingerprint Card (required if this is your first MN license)
✔ Application Fee
o First-time applicants: $ 90.25 (check or money order) payable to “PELSB”
o Applicants hold an MN license (substitute, limited, etc.): $ 57.00 (check or money order) payable to “PELSB”
✔ Signed Section 6 “Verification of Completion of a State Approved Licensure Program” --- OTE will email you the signed verification form (Step A above). Print Section 6 and include it in your packet
✔ Transcripts--- OTE will email you the copies. Print your transcripts and include them in your packet
✔ Parent Education Learning Contract--- OTE will email you a copy of your contract. Print your contract and include them in your packet
✔ Any additional attachments required for your license--- OTE will email you, if any. Print and include them in your packet
✔ A printout of an email from OTE* on which the signed verification of completion form and transcripts, other items are attached.
Please print the email from OTE, and include it in your packet. *You must print and include a print out of the actual email you receive from the OTE in your application packet to PELSB. Per a directive from PELSB, transcripts that are submitted without the email indicating they have been forwarded from the teacher preparation provider will not be accepted.
Please send only completed licensure applications to PELSB. Incomplete applications will cause a delay in processing your license. It is the licensure candidate’s responsibility to ensure all materials are included in the application.

Option 2: Use PELSB online application: this option is only available to the first-time MN license applicants
Your application packet should include the following items:
✔ Complete PELSB online application: https://public.education.mn.gov/CELApplication/
o Must have a Google account. Your U of M email account should work fine
o Pay an application fee using a credit card
✔ License Application Cover Page/Document Checklist from PELSB online application system. Print the cover page/document checklist from your PELSB application account, and include it in your packet.
✔ PELSB Fingerprint Card (required if this is your first MN license)
✔ Signed Section 6 “Verification of Completion of a State Approved Licensure Program”
OTE will email you the signed verification form (Step A above).
Hand-write your PELSB online process number on top of Section 6 (your process number is listed on the License Application Cover Page/Document Checklist*.)
Print Section 6 and include it in your packet
✔ Transcripts--- OTE will email you the copies. Print your transcripts and include them in your packet
✔ Parent Education Learning Contract--- OTE will email you a copy of your contract. Print your contract and include them in your packet

Additional Notes
Do I need to have my fingerprints taken for my MN license?
Fingerprints are required for the first time MN license applicants.
To request a fingerprint card, email pelsb@state.mn.us and include your full name and mailing address in your message. The subject line of the email should be “Fingerprint Card Request.” If you use a fingerprint card from a local police station, make sure it has the sky blue border.
Some Minnesota counties have resumed fingerprinting. Please reach out to see if this is available in your area. Some third-party sites may also be available for fingerprinting. Make sure to call ahead to find out if you need an appointment in advance before going to a fingering location. Also, ask the site regarding specific preventive measures they are taking to ensure the safety of you and their staff. If you do not feel comfortable, try another location. Refer to the attached fingerprint instructions document for how to complete your fingerprint card.
Transcripts
We will be emailing you copies of your transcripts. You do not need to provide your transcripts to us, as we should have access to your transcripts.
Please make sure you sign the transcript release form
Applying for a license outside of "Minnesota"
You must apply for your MN license, even if you are leaving MN to teach in another state!
