Required Health Physicals Before Participation

The Physical Form provided below must be turned in to Mr. Corral before the sport you are participating in begins its season. You only need one form even if you are participating in more than one sport.

Student/Athlete Fees

The athletic fee for the 2018-2019 school year will be $25 per sport played. Fees must be paid in full for a student-athlete to receive enrichment credit for playing each respective sport.
