A note from the nurse:


I have enjoyed helping to care for the health needs of your children this year! There are many things to think about as we approach the last week of school. Here are just a few reminders from the clinic:


End of School Year Medication Pick up – Please pick up ALL medications from the school clinic by 3:00pm on Friday, May 31st. If medication is not picked up from the clinic by 3:00pm on May 31st  it will be considered abandoned. Abandoned medication will be destroyed in accordance with local, state, and federal laws. We do NOT keep any meds in the clinic over the summer.

New Health Forms/Meds for the 2024-25 School Year - With each new school year, it is necessary to update your child’s Health Forms in the school clinic. All students with medical conditions (ie Asthma, Allergies, Seizures, etc.) must submit new health care plans each school year.

If you plan to bring medications to the clinic next year, you must complete a Request for Administration of Medication form with each medication turned into the clinic. If the medicine is over the counter, only a parent's signature is required. However, if it is prescription medication, or needs to be administered for more than 2 weeks, the form must also be signed by a physician. Please see the district website: Departments Health Services/Medications at School  and then resources on the top right hand side of the website for links to these medication forms. 

If you would like for your student to carry emergency medication, such as an inhaler, epi-pen, or glucagon/diabetes supplies, the physician must indicate on both the Care Plan and the Authorization for Students to Carry form that the student is trained and capable of carrying and self-administering the listed medication. The Authorization for Students to Carry form requires physician, student, and parent signatures.

 Immunizations: By State law, your child must have a minimum number of required vaccinations to attend school. SWCSD requires all students to be immunized (or in the process of being immunized) against poliomyelitis, measles (aka rubeola), diphtheria, rubella (German measles), pertussis, tetanus, mumps, and other diseases legally designated in accordance with State statutes, unless specifically exempt for medical or other reasons. SWCSD requires that students enrolled in grades seven through twelve (12) be immunized against meningococcal disease in accordance with the Ohio Department of Health. Parents/guardians MUST provide records that indicate the vaccinations are complete or are in process. Please be sure to ask the health care provider for an updated vaccine record for the start of the new school year. This record can be emailed to the nurse pme1205@swcsd.us, sent into the school with your child or faxed.

Keep your Infinite Campus Parent Portal information updated to get text, phone, and email updates!

Infinite Campus (IC) is the District’s Internet-based student information system. Important features of IC are the Parent Portal and NEW Mobile App. Through the IC tools, parents are able to access their child’s information, such as class schedules, attendance, academic reports, transportation information, coursework, and put funds in their child’s food service account online. Parents can also designate how and where they wish to receive emergency communications, attendance calls, general announcement messages, and e-mail messages from teachers, the school, and the school district. New accounts need an activation key. The district will be mailing those out in August, so save the letter if you need to create a new account.
