In addition to Final Forms registration, you must have an updated OHSAA physical form completed and signed by a medical professional and you. Specific to wrestling, high school student-athletes have to undergo hydration testing to determine their lowest allowable weight class for the season. The parental consent form is in the below files if needed. This will need to be signed prior to our testing date at a local high school in the middle of November.
Skin infections (ringworm, impetigo, etc.) are relatively rare, but they are a part of participating in a contact sport such as wrestling. In the files below is the skin form that needs to be signed and dated by a medical doctor (cannot be signed by nurse practitioners or chiropractors). The date and time listed on the form is when the wrestler will be allowed to wrestle again in practice and competition. It is important to get any suspect skin lesions seen by a medical professional as soon as possible so as to prevent their spread.
