SCHOOL HEALTH EXAMS AND UP-TO-DATE IMMUNIZATIONS REMINDER:

· All new 3 and 4 year old preschoolers, all kindergarteners, all sixth graders, and new students from out of state are required by the State of Illinois to obtain a school physical and be up-to-date with immunizations by October 15, 2017.

· In addition, kindergarteners and any students newly enrolled in Illinois schools are required to have vision exam by October 15, 2017.

· All 6^th grade students must show proof of receiving the Tdap vaccination and Meningococcal vaccination (Meningitis) by October 15, 2017.

· All kindergarteners, second graders, and 6^th graders are required to have a dental exam by May 15, 2018.

The completed physical forms with immunizations, vision exam forms, and proof of Tdap and Meningococcal vaccinations are due in our office by October 15^th. Failure to obtain these necessary exams by their deadlines may result in exclusion from school until they are complete, per the State of Illinois. Please send completed exam forms to the school office as soon as possible after the exam.

NOTE: Parents, be sure to fill and sign out the portions of the forms for parents. The health form has a questionnaire and a place for the signature on the back. This is required to be completed and signed.
