All DSG forms are due on April 1 of each school year. DSG forms are submitted as a complete packet. A complete packet includes the DSG summary sheet (table of contents) and all verification forms collected for the year. Students should make a COPY of the packet before submitting the packet on April 1.
1. To join DSG, a student should submit a completed Student Application.
2. Each time a student volunteers, the supervising adult signs the Verification Form.
3. The Summary Cover Sheet is submitted on top of the complete packet on April 1.
4. People and organizations requesting DSG students should submit the Site Coordinator Request Form to Alfred Jones and Wunsche High School.
