PRESCHOOL / TK REQUIREMENTS

 ALL students entering Preschool/TK are required to have the following: 

• Up-to-date immunization certificate.   

  • The Iowa Immunization Law requires that parents of all children attending schools in Iowa provide written proof of immunizations. No child may attend school without a Certificate of Immunization, except under circumstances outlined or specified in the State of Iowa laws or codes. 

  • Form attached below.

• A physical completed within the last year.

  • Form attached below.

KINDERGARTEN REQUIREMENTS

ALL students entering kindergarten are required to have the following: 

• Up-to-date immunization certificate (form attached below).

  • The Iowa Immunization Law requires that parents of all children attending schools in Iowa provide written proof of immunizations. No child may attend school without a Certificate of Immunization, except under circumstances outlined or specified in the State of Iowa laws or codes.

• A Certificate of Dental Health completed by a dentist, dental hygienist, physician, physician assistant, nurse practitioner or nurse.

• Lead screening. Iowa law (House file 158) requires evidence of a blood lead test when children enter school. Please see the Iowa Department of Public Health website for more information.

• Vision exam - The nurse will perform vision screening in November.  If you have eye insurance please see your provider.

3RD GRADE REQUIREMENTS

ALL students entering 3rd grade are required to have a vision screening. 

• The nurse will do a screening in November.
