MARCHING BAND DOCUMENTS:
UIL Physical Form:
Physicals are good for two years. If you are unsure if your band student needs to have a physical, email Mr. Olson.

UIL 8-hour Rule Form:
This form informs families of the UIL regulation that bands cannot rehearse for more than 8 hours per week outside of the school day. This does not include football games, parades and other performances.

2024-25 Band Medical Release:
This form is what we give to medical personnel (EMT's, hospital, etc.) if your band student is in need of medical care while on a band function. If you are having trouble accessing this file, please email Mr. Olson and he will send one to you.
