UIL Physical Form:
Physicals are good for two years. If you are unsure if your band student needs to have a physical, email Mr. Olson.

UIL "8-hour Rule" Form:
This form informs families of the UIL regulation that bands cannot rehearse for more than 8 hours per week outside of the school day (not including football games, parades and other performances). This form needs to be on file at the school.
2025-2026 Band Medical Release:
This form is what we give to medical personnel (EMT's, hospital, etc.) if your band student is in need of medical care while on a band function. If you are having trouble accessing this file, please email Mr. Olson and he will send one to you.
ALL of these forms MUST be turned in by the first day of Band Camp (August 3, 2026)!
