In addition to providing a systemic approach to our transition planning instruction, Reading Community Schools can provide information to parents regarding transition systemically, as well. As information is provided to our families quarterly, please include the following transition planning information along with your progress monitoring. Providing information by grade level will ensure we have provided sufficient information regarding the transition process, as well as, help parents transition their child from being a student to life as independent and productive adults.
