$200 AIA Participation Fee is only due if athlete makes the team

3. ATHLETIC PAPERWORK TO TRY OUT - required for every athlete every year!
Parents must register their student through the website "Register My Athlete." Parents will need to create a user account, complete forms, and upload them into Register My Athlete (RMA) prior to tryouts.

Turn in to Coach - last page signed and dated by parent and athlete
STUDENT SELF-TRANSPORT FORM –
Complete form if your player will be driving himself
STUDENT RELEASE FORM –
Turn in to Coach with all potential game dates on the sheet
Complete form if your player will be going home with parent from an away game instead of the bus
