SUBSTITUTE REQUEST FORM (less than 48 hours)

Please use this form to request a substitute when requesting a day off with less than 48 hours from the date of your absence (as you used to do with the substitute caller line). If the date of your absence is more than 48 hours, please use the Employee Portal for your request.

2024-2025 Absence Request for Adjustment Open now

Use this form if you need to change a previously submitted absence request to 1/4 or 3/4 of a day or if you need to cancel or adjust a requested day in any way

FMLA Form Contract Period-07/01/2024-06/30/2025

Complete this form when you need to apply for FMLA/NJFLA to see if you would qualify. Please use the links that may apply to your specific need. When requesting Maternity FMLA/NJFLA Leave we would also like a letter with an explanation of the time you intend to use once you have completed the Google Form.  Example of a letter.

Have this form completed by you and your medical provider prior to returning to work from an FMLA Leave or any extended absence due to your illness.

Worker's Comp Procedures

This form must be completed for schedule B/C positions to submit your invoice to the business office for payment.

WORKSHOP & TRAVEL REIMBURSEMENT FORM 

Workshop Travel Reimbursement Memo

Please review the Travel Reimbursement Guidelines if you will be seeking mileage or another reimbursement.

NOTE: Any workshops with a total cost of $150 or higher need board approval and must be submitted in a timely manner prior to the board meeting before the workshop!

Complete this form to submit a professional development/workshop request for approval. Remember to have any documentation available in digital format so you can upload it to the form and remember to submit your absentee request for the date. If you will be a chaperone for the Senior Class Trip the Workshop Request must be completed as with any other travel reimbursement.

Workshop Reports: due after workshop attendance

Must be completed in a timely manner after attending a workshop. 


Request for Approval of Course for Credit-Now Open

This form must have approval prior to your starting the class. If you do not have this approved it can have a negative effect on Tuition Reimbursement.

2024 2025 TUITION REIMBURSEMENT  -Now Open

Please complete and upload your transcript and Proof of Payment. Please have your Transcript and POP ready to upload when filling this form out. This needs to be submitted by June 15th.
