Immunizations

    

Parents or guardians must provide the school with the child's immunization history at the time of enrollment.  This history includes the month, day, and year for each vaccination.  Children will not be permitted to attend classes until the immunization history has been provided. If the student's shot record is not current at the start of school, the student will be excluded from school after 14 school days of not providing current immunizations. 

The Ohio Department of Health has released new immunization requirements for the 2016-2017 school year. These new requirements affect students entering the 7th grade and 12th grade in the upcoming school year. 7th grade must have ONE Tdap AND MCV4 (meningococcal) vaccine prior to starting school. 12th grade must have TWO MCV4 (megingococcal) vaccines prior to the start of school.

The Montgomery County Public Health Department has made us aware that a case of Measles has been identified in Montgomery County. Measles is highly contagious. To date, no cases have been identified in our district.
