Attention Parents/Guardians: The pre-participation physical exam must be completed by a health care provider who has completed the Student-Athlete Cardiac Assessment Professional Development Module.
Sports physicals cannot be accepted if this is not completed by the health care provider.

Pre-Participation Physical Exam Form
Once completed, the Preparticipation Physical Evaluation form will remain on file at your primary care doctor's office.
The only form to be submitted to the school is the Medical Eligibility Form.
The Medical Eligibility Form must be completed by your primary care doctor and emailed to
athleticshealth@millstone.k12.nj.us
For a physical to be considered current for Fall Sports, it must have been done on or after 9/1/23.
