2026/2027 SCHOOL YEAR
State regulations require that students get their physicals from their private physicians. The examination must be documented on the Athletic Pre-Participation Physical Evaluation and remain at the student athlete's medical home (doctor’s office).
The only form that needs to be submitted to the school after the physical examination is completed is the Medical Eligibility Form, which must be signed and stamped.
