COMMUNITY SERVICE

At Middletown Middle School, students may accumulate community service hours in various ways during their 7th and 8th grade years. Students who have a minimum of 10 hours will be recognized at our 8th grade promotion ceremony. Down below, you'll find our Community Service form available to print. Students may use another form if they desire as long as it lists a description of what they did, how many hours, and signed off by an adult who supervised the hours (not a parent/guardian). Community Service is not required for middle school. We do recognize those students at 8th grade promotion with 10 or more community service hours.

In order for hours to count towards their middle school hours, we must have a completed form submitted to the office.

DISCIPLINARY PROCEDURES FORM

This form is to be signed by both student and parent/guardian that they understand the disciplinary procedures and student behavior.

ENROLLMENT FORM (ENGLISH AND SPANISH)

The enrollment packet is available to download and print. Please make sure to fill out all sections. If you have any questions or concerns, please contact your school's secretary. When submitting the enrollment packet, the appropriate immunizations will need to be present before the student can be enrolled.

FREE AND REDUCED LUNCH PACKET AND INFORMATION

Starting on the first day of school, ALL students will be charged for their meals unless you have completed a Free and Reduced Price Meals Application. Please take the time to fill it out even if you do not believe you are eligible or you do not think your student will participate. This is important as it helps with district funding. Your application is completely confidential and your information will be protected. Please return it to your school office or the District Office as soon as possible.

ACKNOWLEDGEMENT OF PARENT/GUARDIAN NOTICE OF ANNUAL RIGHTS

THIS IS PART OF THE ENROLLMENT PACKET! This packet includes the following documents: Annual Notice to Parents, Free and Reduced Meal Packet, Annual Notification of The Uniform Complaint Procedures, and Williams Uniform Complaint Procedures. MUSD requests that parents/guardians fill out the entire packet, especially the Free and Reduced Meal Packet regardless of whether you believe you qualify or not. Please detach, sign, and return the requested pages to your student(s) school indicating that you have been notified. If you have questions or concerns, please contact your student's school.

MUSD VOLUNTEER PACKET

This packet is for parents/guardians to become Cleared Volunteers with the Middletown Unified School District. Parents/Guardians are required to fill out the packet and submit it to the District Office. Once the packet is submitted, they will need to be fingerprinted and have a TB test completed. For questions or concerns, please contact the District Office.

STUDENT TECHNOLOGY AGREEMENT

This form is to be signed by both student and parent/guardian that they understand the technology use procedures and expectations.
