Please note that forms are only valid for 60 days and must be picked up by a parent/guardian.

Who Needs This Form?
Students under 18 who are applying for a driver's permit need a Driver's School Compliance Verification Form (No Pass/No Drive Form). You do NOT need this form if you are taking your road test.
To qualify, students must meet the state's attendance and academic requirements for the previous completed semester:
Fewer than 9 unexcused absences and
Passing 6 out of 8 classes, or the equivalent
Important: Students must bring the original paper form with West Jessamine High School's embossed school seal to the permit test. A downloaded or printed copy of the form will not be accepted.
Students who have graduated or are 18 or older do not need this form.

Submit the request form below. The attendance office will review your attendance and grades and prepare your form. If you DO NOT qualify, you will be contacted via email.
Please allow up to two school days for processing. Requests will not be processed when school is not in session and West Jessamine High School is closed.
If you are turning 15 and want to take your permit test on your birthday, please submit your request 1–2 weeks in advance and your form will be completed and post-dated for your 15th birthday.
Forms expire 60 days after they are issued.
A parent or guardian will need to come to the West Jessamine High School main office to sign for and pick up your form (since this is a legal document).
Forms will only be available between 9am and 3pm for pick up.
