Volunteer Driver

Are you a volunteer who will be driving students in your personal vehicle?

You will need to have the following clearances:

#1 Volunteer Application 

• Once you fill out the application I'll send  you the fingerprint form with instructions

#2 TB Risk Assessment or negative TB test result

• Online TB Form

#3 Fingerprint clearance (this can take up to two weeks to process)

#4 Form 28.02 Permission to Transport Students Using Personal Vehicle

#5 Copy of Driver's License

#6 Copy of Insurance Policy/Declaration showing a minimum of $100,000 Liability Coverage

Volunteers driving students in their personal vehicles must fill out the driving form each school year.
