All athletes will need a new athletic physical for the 2025-2026 school year.
All athletic physicals must be dated April 1st, 2025 or after to be valid for the 2025-2026 school year.
Students who transfer to a CISD school
-Must obtain a copy of their physical from their previous school
or will be required to get a new physical for this year.
All students participating in athletics within CISD will need to fill out our athletic forms which can be found at the following site.
Form for the 2025-2026 school year will not be available to fill out until July 1, 2025
