Immunizations
By law, a student's immunizations must be current, in progress or the student must have a notarized Conscientious Objection (original form will be kept on campus).

Affidavit forms must be brought to campus with the student on the first day of attendance. This form must be the original blue form from the state and notarized within the state of Texas. This form will be in good standing for 2 years, and after that you will need to provide a new updated and notarized form to replace for your student. The form can take several weeks after requesting to receive from the state, so request several forms early! Click here to request the form for immunization exemption.
