5th Grade Camp
The campus nurse will go to camp with all 5th grade students and be availiable to them during their stay.

MEDICATIONS
NO MEDICATIONS WILL BE RECEIVED AFTER THE DUE DATE.
Exceptions:
Epi-pen if you only have one and need to keep it at home until the day of camp.
Inhaler if you only have one and need to keep it at home until the day of camp.
Newly prescribed medications (antibiotics, etc.)
NO EXPIRED MEDICATIONS WILL BE GIVEN.
Parent must must make sure non-expired medications are provided.
The only medications that do NOT require a prescription or your child's physician's signature (BUT STILL REQUIRE A COMPLETED MEDICATION FORM):
Advil, Motrin (ibuprofen can be the only active ingredient -- review label)
Tylenol (acetaminophen can be the only active ingredient -- review label)
Benadryl (diphenhydramine can be the only active ingredient -- review label)
TUMS
ALL OTHER MEDICATIONS MUST HAVE PHYSICIAN'S ORDERS OR A PRESCRIPTION LABEL -- even if it's only an "over-the-counter" medication.
I would greatly appreciate it if you would send the medication in as small of a bottle as possible (no 500-count Advil, please)!
Scheduled medications will be administered before breakfast and/or after dinner. Please talk with the nurse if you have any concerns.
PLEASE ENSURE TO SIGN ALL FORMS
Especially the form on the camp packet to give consent for medical aide and transportation in the event of an emergency at camp.
