Physical Examinations are REQUIRED for

• • All NEW STUDENTS to the Florham Park School District (Not required for students entering directly from Brooklake 5th gr.)

  • SPORTS Participation - See Navigation bar "Athletic (sports) forms" for the mandatory NJ state form Note: Fall sports forms are due Aug. 6

  • Physical Examination for SEVENTH GR students is recommended by the NJ State Board of Health and our school physician to update student's school health records and ensure we are aware of student's current health status.
