Request for Nurse to Administer Medication form. The School Nurse MUST administer ALL medications except those for life-threatening conditions (see below *areas). Parent/guardian and doctor are to complete this form.
Asthma Treatment plan with orders - All students with Asthma must have a Treatment Plan per NJ mandates.
LIFE-THREATENING FOOD ALLERGY IHP ( Individualized Health Care Plan) and important Instructions an letter for the Allergy Plan.
Seizure Action Plan- Have your physician complete this form and contact the School Nurse to develop a Individualized Health Care Plan to be based on your physician's orders.
For students with DIABETES, contact the School Nurse for an Individualized Health Care Plan to be based on your physician's orders.
* Only students with life-threatening conditions may carry/self-administer their own medication (asthma, severe allergies, diabetes)
