Grant CCoT (County Communities on Transition)

The Grant County Communities on Transition (CCoT) is a community of key stakeholders at the county level which work together to impact outcomes for youth with disabilities in transition to adult life.  Our vision is that all youth with disabilities in Grant County will transition to a healthy, full and meaningful life.  The Grant CCoT members includes county level staff from the following organizations:

Aging Disability Resource Center (ADRC)

Cooperative Educational Service Agency (CESA 3)

Division of Vocational Rehabilitation (DVR)

Hodan Community Services

Include, Respect, I Self-Direct (IRIS)

Independent Living Resources
