Revised: February, 2020
Policy
The Company strives to be fully informed regarding the reasons for, and circumstances surrounding, team member separations. Accordingly, departing team members are solicited via exit interviews and/or questionnaires in order to obtain their honest and thoughtful appraisals of the Company’s operating practices, policies, benefits, compensation, etc.
Accountability
If requested by a departing salaried operations team member, an exit interview will be conducted by the Area Supervisor or a member of the Human Resources department. The Human Resources department also sends a request to all separated personnel to complete an online "exit interview" survey.
Practice
Confidential Exit Questionnaires
Exit interviews are to be documented by the Company representative who conducts the interview. Completion of a confidential Exit Interview Questionnaire is the minimum necessary documentation. All handwritten notes, whether on the questionnaire form or not, should be maintained as part of the permanent record. Instances where issues such as pay, benefits, or alleged mistreatment are noted may require additional investigation and/or documentation.
When completed, forward a copy of the confidential Exit Interview Questionnaire to the Human Resources department to be retained as part of the former employee’s permanent record.
Online Survey
The Human Resources department has created an online, automated exit interview process. All separated personnel receive a request to complete an online survey regarding their separation. The survey responses are maintained in a database and monitored to determine if any corrective action is necessary.
