Step 1: Read and understand the 2025-2026 Parent Handbook that is attached below prior to registration.

Step 3: Complete the necessary forms (See uses of forms below to determine need) and return to a SVE BASE staff member.
Medication Release Form: For children who receive medication during program hours. Must be signed by Primary Care Physician.
Extra-curricular Release Form: For children who participate in extra-curricular activities while attending the program. Ex: Chess club, Lego club, Band, orchestra, tutoring, etc.
*Please allow a minimum of 48 hours for SVE BASE to approve enrollment.
