The School Based Health Center is located to the right of the Main Office. Students who are ill or injured at school need to go to the school nurses first. The student will then be referred to the SBHC from the Nurse’s office if indicated.

The Rogers Park School Based Health Center (SBHC) is the third SBHC to open in Danbury Schools. It is a licensed clinic that provides medical and mental health services to students who have a SBHC consent on file. A parent or guardian must complete a consent which gives the staff permission to work with your child. The services are free, even to those with insurance. It is open from 8AM to 2 PM when school is in session.
We do not replace your medical providers, but strive to keep students healthy, in school, and successful by providing on-site medical and mental health services. An added benefit is that parents can often remain at work while their child is being seen at the SBHC.
Consent forms are available in English, Spanish, and Portuguese. Please fill it out in ink and sign at the bottom of BOTH pages. Complete the history on the back page. See Privacy Notice.
