Per Ohio Administrative Code, the "Summary of Work-Related Injuries and Illnesses” form specific to each School/Service Building requires posting at each site from February 1st through April 30th. This form is to be posted “in a conspicuous place where notices to employees are customarily posted”. This is an “employee” information document and need not be in a location specific to non-employee individuals (i.e., visitors, students, etc).
Each school/building main office shall:
1. Find and click on your school/building location from the following list:

2. Print off the form (OSHA 300 AP log) for your school/building
3. Post in a location accessible to employees from Feb 1st – April 30th

Notes relative to the Summary of Work-Related Injuries form:
This form is to be posted from Feb 1st – April 30th of year following the year covered by the form. Note that after April 30th please take them down and toss; the Office of Environmental Health and Safety maintains copies for the required time period as specific in the code.
Not all “injuries and illnesses” are required to be listed in the summary, only those that fall under the definition of “recordable” as defined by OAC and PERRP; following the PERRP guidelines, my offices decides which are “recordable” and includes these in the summary. If you have any questions on the logs, including injuries that may not be posted, please contact the Office of Environmental Health and Safety.
All establishments require completion of this summary, and posting for the Feb-April timeframe, even if no recordable injuries occurred.
In addition to the summary form sent to your school/building location (Form 300 AF), there is a “Log of Work-Related Injury and Illness” (as they relate to recordable injuries). This form is known as “Form 300P”. The Office of Environmental Health and Safety manages maintenance of this log on an ongoing basis; access to this log is available via request to my office, in writing by any staff, staff representative or legal counsel.
