The CHS Counseling Department must have written permission in order to release records to any post-secondary institution, military recruiter, and/or scholarship/financial aid organization.
Students fill out the Transcript Release Form. The form is not complete and cannot be processed without both a student signature AND a parent/guardian signature. No electronic signatures will be accepted.
In order to request letters of recommendation, Seniors need to fill out the Senior Brag Sheet Form (linked below). This form helps teachers and counselors write personalized recommendation letters. Recommendation letters are submitted with most college/university applications and/or with scholarship applications.
Different post-secondary institutions have different entrance requirements. Some 4-year colleges and universities may require students to submit SAT or ACT scores. Some 4-year colleges and universities may not require standardized testing and/or may be test optional. Test optional means that the student is able to decide if they want to take and submit any standardized test scores. We have provided a list of tests and test dates below for your information.
ASVAB (Armed Services Vocational Aptitude Battery):
Collegium High School will offer the ASVAB to interested 11th and 12th grade students.

Junior Year
Update your Resume in Naviance.
Take the PSAT one more time.
Complete assessments in Smart Futures.
Establish a "Colleges I’m Thinking About" list in Naviance.
Decide which schools you may want to visit.
Attend the CCS College & Career Fair.
Attend CCS Financial Aid Night.
Complete and upload Junior community service hours.
Consider a summer enrichment program or internship.
Junior Year
Begin visiting schools and gathering further information.
Meet with your counselor for a Post-Secondary Planning conference.
Begin taking standardized tests (SAT/ACT/ ASVAB).
Attend a Spring College Fair.
Before leaving for summer break, begin asking at least two teachers for recommendation letters. You will not be able to upload these letters into Naviance before Senior Year.
Consider a summer enrichment program or internship.
Continue visiting colleges and gathering information.
Senior Year
Complete the Brag Sheet form.
Complete a recommendation request through Naviance.
Register for an account with the Common Application, www.commonapp.org ‐ after August 1
Attend Senior Seminar.
Set up a Senior Conference with your assigned school counselor.
Make a plan to complete your Senior community service hours.
Move “Colleges I’m thinking about” to “Colleges I’m applying to” in Naviance.
Set up FAFSA ID.
Attend Financial Aid Night and FAFSA Workshop.
Complete the FAFSA with your parents/guardians.
Review financial aid packages thoroughly.
Senior Year
Continue to research scholarships and apply.
JANUARY: Request mid-year reports as required by your school.
Complete the FAFSA if you haven’t already.
Review your acceptances and revisit campuses if necessary.
Talk to your parents, counselor, and teachers about making your final decision.
Notify the school you plan to attend and send a deposit by May 1st.
Accept the financial aid award letter by following the instructions given by the school.
Update your Naviance account with your acceptances & notify your counselor where you plan to attend.
Complete and Upload ALL service hours by the end of May.
