Guest Form

Guests must have a guest form filled out for them in order to be approved to attend prom. This needs to be printed off and brought to Ms. McGlamery or given to Mr. Rogers; a background check will be conducted. Guests must be attending high school, and can not be over the age of 20.

If a student does not attend a high school/college, then their employer must fill out the form before being turned in.

Guest form must be turned in by Friday May 6, 2022. There are NO exceptions. If the guest form is not turned in, the guest will not be attending prom.
