At times, medication may need to be administered to your child during school time. Please, meet with the school nurse to discuss your child's specific needs and to review the medication being administered. The Medication Consent Form provided below gives consent and will need to be completed to authorize administration.

The top portion of the form asks for student-specific details. Please, remember to include any allergies.
The "Medication" section provides instructions about giving the medication. "Dates to Be Given" could be specific dates (i.e., 9/20/21-9/27/21) or marked as "throughout the school year" if the medication will be needed for the entire school year. The "Time" is when the medication should be given each day. A prescriber's signature is required for prescription medications.
For the "Authorization" section, a signature from a guardian is necessary. Remember to provide medication in a pharmacy-labeled container and to meet with the school nurse to review your student's needs.
