Discuss Parents’/Adult Student’s Educational Concerns
Review Classroom Progress (e.g. General Education Teacher)
Progress Towards Previous Goals & Grades
Review Assessment Results and Present Reports (Initials and Triennials)
Address Eligibility for Special Education and Related Services
Present Drafts of Proposed New Goals
Review Placement Recommendations
Review Individual Transition Plan pages 1 & 2. HS Counselor to discuss classes (High School, Adult Transition, 8th Transition for 14+).
Review Diploma Track vs. Certificate of Completion (7th grade and up)
Review Behavior Intervention Plan (if applicable)
Review Service Recommendations, Accommodation/Modification, Statewide Assessments Page, % G.E. Participation, Extended School Year, Transportation, Special Factors page and all other IEP Sections
Discuss Programs/Potential Harmful Effects and Determine Educational Placement
State the District’s Offer of a Free Appropriate Public Education (FAPE), including # of minutes to be serviced.
Offer to Review Notes Page with the Team
Obtain Signatures from ALL Participants, Consent, Medi-Cal, and Parent Involvement (in person or electronically)
Verify Request for Paperwork to be Translated (if necessary)
Number Pages (if in-person)
Distribute Copies of IEP Documents and Reports to Parents (if in-person)
