Step 1: Site receives completed Home Hospital Referral Form and Release of Information Forms with signatures and dates.
**Note: If the student has an IEP or 504, the IEP or 504 team must convene a team meeting and recommend that Home Hospital Teaching is an appropriate placement. The IEP/504 Case Carrier must sign off on Part 3 of HHT Referral Form. Our new Director of Special Education-Elementary, Dr. Stephanie Richardson (richardson_stephanie@ausd.us), and our new Director of Special Education-Secondary, Mrs. Debbie Stone (stone_debbie@ausd.us) will assist families of students with IEPs that have a Home Hospital recommendation.
Step 2: Completed forms must be given to your site Nurse. Site Nurse reviews the forms to make sure they are signed/dated, and appropriate. Site Nurse signs off under the Health Service dotted line on the first open spot. Site Nurse scans forms to AUSD Lead Nurse - Damaris Pessina - pessina_damaris@ausd.us
Step 3: Damaris Pessina reviews and approves by signing off under the Health Service dotted line on the second open spot. AUSD Lead Nurse scans the forms to Lorena Labbe- labbe_lorena@ausd.us
Step 4: Ms. Susan Yu will contact the family and set up the Home Hospital Teaching arrangement. Ms. Susan Yu will reach out to teachers to find a Home Hospital Teacher for student. Home Hospital Teaching is extra duty and is conducted outside of the teacher's contractual hours. Please be patient as it sometimes takes a few days to find a teacher willing to conduct Home Hospital Teaching.
The student's doctor should always be the person recommending Home Hospital.
Note: Per CA Ed Code 48206.3, The district in which the home or residential health facility is located is responsible for instructing and educating pupils who must be hospitalized or remain at home due to a temporary but extended illness or disability.
