- The student is referred by: teacher, parent, and/or other school team member.
- The classroom teacher completes the OT Referral Form: https://goo.gl/forms/05JwcHLBbv26hueK2 --(This form will come straight to my email, and I will address it as soon as I can.)
- OT completes an observation and/or screening once parent consent is given (can be written consent by email or via signature on the Building Screening Committee Form 2 (BSC-2).
- Interventions may be offered as a part of Response to Intervention (RTI).
- If the student is eligible for an IEP or 504 plan, the OT will then determine if a full evaluation is needed. OT services on an IEP are a "Related Service." In other words, OT services cannot "stand alone." For a student to receive OT services, they must qualify for support from the Learning Center teacher, the Speech-Language Pathologist, and/or the Adapted PE teacher. There need to be goals and/or accommodations that are appropriate for the OT to support.
- If appropriate, an evaluation planning meeting is then scheduled by the case manager.
- Parent gives written consent for evaluations decided by Occupational Therapist.
- OT completes the evaluation and writes a report.
- Results are shared with the parents/legal guardians and the school team.
- Eligibility for Special Education is determined by the team.
- An IEP is written or reviewed. OT is involved in the development of the IEP.
- Then, if the OT recommends services and the team (parents and school team) agree that the student requires OT services in order to make meaningful progress on their IEP goals, OT services will be included on the IEP.
OT Services on an IEP in PPS are provided through a collaborative team approach in the least restrictive environment. (Example: In general education settings whenever it is appropriate in order to meet the student's needs.) As a Related Service, OT consists of observing, collaborating, trialing tools, adaptations, and strategies to support the IEP goals.
OT Services on a 504 provide consultative services with the teacher for accommodations and modifications only.
