1. Pick out a case. The case does not have to be rare, it just has to have good radiology-pathology correlation. The online case submission needs to contain radiology images, at least 1 gross pathology image, and at least 1 histology image (according to the website, exceptions for non-neuro cases can be made, however you have to email them). Generally the most difficult part of the case to acquire is the gross images, since pathology does not always take pictures of the gross specimen. 


2.  You also need to submit histology slides, which can either be physical slides or virtual microscopy (with acceptable formats specified on the website). Email the pathologist directly who read your case to acquire the physical slides or virtual microscopy.  Since either physical or virtual slides are accepted, just ask the pathologist which is easier for them to prepare. If your case is from CCHMC, you can also try emailing pathology@cchmc.org for information.


3. The online case submission requires you to upload a few representative radiology images. In addition to this, you need to submit a DICOM file containing the entire imaging study (for example all images axial/coronal/sagittal planes of a CT abdomen/pelvis). You can export the images to a DICOM file using the Export tab in McKesson. You can also contact Karen Putnick for a DICOM CD containing de-identified images of the study you need. To acquire the DICOM images from CCHMC, contact Laurie Perry (Lead Technical Specialist) at Laurie.Perry@cchmc.org


4. It is required that every identifiable piece of information from the radiology/pathology/operative reports is removed (no patient information, doctors' names, phone/fax numbers, addresses, institution names, etc.).
