Building Use Request

If you are interested in using our facilities, please fill out the "Building Use Request" form below and return to FCC via mail or email to office@fccoshkosh.org.

If you are interested in using our facilities for vocal or instrumental activities please fill out the "Building Use Request", as well as the "building use policy music". We then ask that you review the "Guideline for Music requests during COVID" and sign the Music rental COVID agreement.
Return the "Building Use Request", "building use policy music", and "Music rental COVID agreement" forms via mail or email to office@fccoshkosh.org.
