Commitment to our Clients:

- All treatments provided by ASM will last for the specified time.

i.e. a 45 minute treatment will last 45 minutes.

- All clients must complete a consultation during treatment session 1, this will include any appropriate testing/ assessment and will last up to 15-20 minutes, followed by a 25-30 minute treatment.

- Treatments will only be conducted if ASM deems the treatment safe for the client to receive.

- GP/ Consultant permission may be requested before treatments can be provided.

Cancellation Policy:

- A booking is confirmed once ASM and the client have both agreed a date, time, duration and location for the treatment.

- If a cancellation is made more than 24 hours before a treatment, the client will not be charged.

Within 24 hours:

- If cancellations are made with less than 24 hours' notice of your booked appointment time, you will incur a one off charge of £20 payable by bank transfer within 7 days. No further appointments will be scheduled until the fee has been paid.

No Shows:

- No shows or no notice cancellations will incur a one off charge of £20 payable by bank transfer within 7 days. No further appointments will be scheduled until the fee has been paid.

Cancellation & No Show Fees:

- The cancellation fee must be paid within 7 days.

- The cancellation fee may be waived at the therapist discretion.

- ASM has the right to cancel a treatment by giving 24 hours' notice.

- ASM has the right to cancel/ terminate a treatment without notice if the room or environment is unsuitable for the treatment to be provided or if ASM is taken ill.

- ASM has the right to cancel/ terminate a treatment without notice if the client is intoxicated, inebriated of if the therapist deems a clients behaviour to be inappropriate or sexual.

- ASM commits to re arranging treatments to a new time and date within two weeks of the cancelled treatment where possible.

Late Arrival:

- A late arrival to a treatment will still require the treatment session to end at the scheduled time, meaning unfortunately the session will be shorter. Your treatment time is reserved for you and you only and other people may be scheduled after this time. Please note the original treatment cost will still apply.

Payment:

- ASM accepts payment in cash, cheque, bank transfer or through card payment.

- Invoices will be sent to clients who do not pay for or who cancel treatments, for clients whose treatment is terminated.

- Invoices must be paid within 7 days of the issue date.

Vouchers:

- Vouchers can be used as payment towards any available, advertised treatments.

- Vouchers are only valid for one person and for one use.

- If the whole amount of the voucher is not used, another voucher for the remaining voucher sum will be issued.

- Vouchers are only valid up to and including the expiry date.

- Refunds are not available for vouchers already provided.

Privacy:

- Consultations and client treatment details will not be discussed with anyone other than the client unless the client is under the age of 16 or has a care worker or guardian.

- Consultation forms and treatment logs will be kept on file securely by ASM for 7 years following the final treatment date (7 years after 18th birthday for a minor).

- All clients must complete and sign a consultation form prior to any ASM treatment and can request access to their personal information and treatment log at any time.

- By signing the consultation form, clients are giving clinic administrative staff access to the document to contact them via telephone, text, email, social media or post.

- By signing the consultation form, clients are giving ASM permission to hold records and data about them. This data will only be shared with other industry professionals (Physio's, GP's etc.) if absolutely necessary.

- Personal information will not be shared with any other third parties.

Young Clients (U16):

- Clients under the age of 16 must be accompanied by an adult.

- The adult must give permission for the treatment to be performed.

- The adult must sign a separate consent of treatment form.

- The adult must be present in the room throughout the treatment.

Website/ Facebook Content:

- Any exercises shown or shared on ASM's facebook page or website are done at a persons own risk.

- Exercises should not be attempted if you have any known pre - existing injuries.

- Exercises should be stopped immediately and not repeated if they cause any discomfort or pain.

- If a person has any questions regarding any of the information on ASM's facebook page or website they should contact ASM directly, this must be e mailed to the following address;

- Website/ Facebook content will be authorised and managed at ASM's discretion, including testimonials.

- Photographs will only be displayed with the consent of the people in the photograph.

- Photograph consent may be obtained by ASM verbally in the first instance, or in writing at the clients request.

Client Feedback/ Complaints Procedure:

- ASM is happy to receive feedback and if a client wishes to provide feedback or make a complaint regarding any of ASM's services, this must be e mailed to the following address;

- Feedback/ Complaints must include the date, time and location of the treatment/ incident, the full name of the client, details of the feedback/ complaint and a desired outcome following the feedback/ complaint.

- All feedback and complaints will be taken seriously and a response will be provided within 28 days.
