In order to be considered for the Work-based Learning Program at Carrollton High School, students must complete an electronic application.   Please submit this copy to Mrs. Austin.

Below is the Parent/Guardian Acknowledgement Form.  For your application to be considered, please click in top right-hand corner then print, sign, and return to Mrs. Austin.  You may scan and then email form to kasey.austin@carrolltoncityschools.net.  Your electronic application will not be considered until this form has been received.  Please do not hesitate to contact Mrs. Austin should you have any questions.
