Health Assessment Information
All kindergarten students must complete the new NC Health Assessment Form to enroll in a NC public school. You may access and print the form at the link below. Parents will have 30 calendar days from the first day of school or the day of enrollment to complete the form. Please contact your school if you have further questions.
