We know things get forgotten sometimes. So we've come up with a way to help out with that. If your member has forgotten to bring home the slip, you can click on the event links to the right and print off at home to send with them to the event. The online form below is only to be used as a last minute option when the student forgets the signed form at home and doesn't have it the day of the event.
