Beginning of the Year Forms - DUE 8/28/26!

Please print, complete, and turn in all necessary forms. These forms will also be available in hard copy at the YHS Office (Monday-Friday 8AM-4PM).

Free/Reduced Meals Application - If your child(ren) qualified for free or reduced price meals at the end of last school year, you must submit a new application by August 28, 2026 in order to avoid an interruption in meal benefits.

Student Chromebook/Device Forms - YHS Device Checkout will be held from 8:30am-3:00pm on

August 3rd-August 5th. Forms below will need to be completed and submitted prior to receiving the chromebook.

REMINDER: ALL students will be issued a school chromebook and personal computers will NOT be allowed to access the school network.
