Please begin to organize the following items prior to starting the application process:

Create a Google Email Account: If you already have a Google email account then there is no need to set up an additional account.
Coach's Reference: Speak with the coach of the main sport in which the student will be applying under, and obtain their correct email address. Advise the coach they will receive a digital reference form once your application is submitted. If an incorrect email has been provided, the reference form will not be received which will therefore make your application incomplete. Please ensure you have listed the correct email address for your coach as references are completed digitally. Only ONE coach's reference can be submitted.
Academic Information:
Current Grade 8 Students: Please submit a Grade 7 Final Report Card with your application.
Current Grade 9 Students: Please submit a Grade 8 final report card and your current midterm report card, with your application.
*If you are NOT currently a student at St. Maximilian Kolbe CHS, you must ALSO submit a copy of your credit counselling summary. (This can be obtained from your school's guidance department).
Current Grade 10-11 Students: You will require a copy of the following:
This year's Semester 1 midterm report card.
Final Semester 2 report card from previous academic year.
*If you are NOT currently a student at St. Maximilian Kolbe CHS, you must ALSO submit a copy of your credit counselling summary. (This can be obtained from your school's guidance department).
5. Payment: $50 NON REFUNDABLE fee upon submission of application. Payments will be made through "School-Day." Information on payment submission will be on the application form.

PLEASE ENSURE YOU PROVIDE US WITH THE CORRECT EMAIL ADDRESS AS WE MUST SEND HIM/HER A REFERENCE FORM. PLEASE CHECK IN WITH YOUR REFERENCE TO ENSURE THEY HAVE RECEIVED THE EMAIL 48 HOURS AFTER YOU HAVE SUBMITTED YOUR APPLICATION WITH US. COACHES MUST GET THEIR REFERENCES INTO US BY THE DEADLINE DATE - DECEMBER 13, 2024 (11:59 PM).
