Community Service Hours

Students can now submit their community service hours electronically to our guidance secretary,  Ms. Muktar:   fmm.guidance@ycdsbk12.ca

Guidelines for submitting community service hours are as follows:

1. Complete the Community Service Hours form in full detail! 

2. Ensure you are only completing ONE FORM PER ORGANIZATION. If you've volunteered at the same location on more than one occasion, you are required to keep track of your hours on a separate sheet with dates and total the hours on the main Community Service Hours form.

3. All contact information for supervisors is up to date and correct.

4.  Email Ms. Muktar (attach your Service Hours form): fmm.guidance@ycdsbk12.ca

Please note - 40 Community Service Hours are required to obtain your Ontario Secondary School Diploma. Without these hours you are at risk of not receiving your Diploma!
