Digital Agreement Forms
*Agreement document below agrees to all documents listed under handbook and forms.

*Forms require parent sign-off. If not completed ahead of time, a parent/guardian needs to be present with student to pick up Chromebook.
Parent/Guardian Agreement (2023/24)
If parent has multiple children at the high school, parent ONLY needs to complete Parent Signature form ONCE - include each child's name on your form. Please be sure to use your own email (not child's) due to the signature web service which links email address with signatures; therefore, student needs their own email to sign their own agreement below.
Student Agreement (2023/24)
If multiple children from same household attend high school, EACH will need to complete and submit his/her own Student Signature form. Students should use their school email (new students can use a personal email, but it must be different from parent email).
Student Information Form (2023/24)
If multiple children from same household attend high school, a separate Student Information Form will need to be filled out for each student.
**Online payment available. Pay your insurance fee online with PaySchools Central (this option should be available starting in August). Insurance payment for the full school year: $40 SCREENSHOT OR PRINT TRANSACTION ID # - bring with you when picking up Chromebook.
