All district medical forms are available on our district wide health portal. You can also access them below on this page. If your child has medication which needs to be given in school, please deliver your child's medication in its original packaging with their name and, if applicable, prescription label attached.
Please bring your child's medications and forms to the front office and ask them to call me to receive them. I ask that you do not put them in a backpack or hand them to your child's homeroom teacher.
A physical conducted within 365 days of the start of school is required for kindergarten registration and for new entrants. For subsequent years, while a physical assessment and immunization record update from a health care provider is not required, it is very much appreciated! Please send in any updated annual physicals at any time throughout the year. Thank you in advance for your help!

Consent for Medication in School
*Please note that in order for medication to be administered in school, even over the counter medications, we require a physician's signature in addition to a parent or guardian. Students may not carry over the counter medications in their backpacks.
*Regarding cough drops: students are permitted to use cough drops in school without a medical provider's note. However, for the safety of all students, district policy requires written consent from a parent or guardian, and the cough drops must be stored in the health office. They are not permitted to be kept with the student or in their backpack.
A note on emergency action plans & medications: Action plans such as FARE forms for food allergies, school asthma plans, or approval for medications to be administered in school are good for one academic year only. Please remember to renew these orders as needed annually. If your child previously required an allergy or asthma plan and a health care provider has determined one is no longer necessary, please let me know the good news so I can update my records!
**All provided medications must be picked up at the end of the school year. We may not store medications, including epipens and inhalers, over the summer.
