From the nurse:
As I look forward to meeting your children in the new school year, there are a few things parents need to know about how things are a bit different in the Nurse’s Office.
1. A parent note excuses your child from gym for one day per event. After that at doctor's note is required. The nurse can excuse your child only once during the entire school year! Please send a note with your child for any unforeseen problems.
2. Your child cannot carry medicine in his/her purse, pocket or backpack. Even over the counter medicine requires a note from your doctor and the medicine is kept in the nurse's office. You may send doctor’s orders and medications in with your child if you wish. But all medications must be accompanied by a note from the doctor and be in a labeled bottle from your pharmacy. Over the counter medication must come with a doctor’s note and submitted with an unopened bottle with your child’s name on it. ALL medications must include a note from you, giving me your permission to administer this medication.
3. By now you should have been notified of the age 11 immunization requirements. Once your child has attained 11 years of age, proof of having received one dose of Tdap and one dose of Meningococcal vaccine is required. Proof of immunizations is due by 14 days after the 11th birthday. If your child attains his/her 11th birthday by the 22nd of August, his/her shot record must be submitted by the first day of school. If possible, please either email it to me over the summer or send it to school on the first day.
